The Let’s Get Down 35211: A High Blood Pressure Management Program Can Help! Let’s Get Down 35211 is a 12-week, community-based high blood pressure management program that provides eligible participants with peer support from trained health coaches, an electronic blood pressure monitor and a group class led by a healthcare provider on strategies for taking blood pressure medication as prescribed.

Ready to sign up?

  • First, have your blood pressured checked at one of our free screenings.
  • If your blood pressure is greater than 130/80 and you live in the 35211 zip code or are a Jefferson County resident who attends Greater Shiloh Missionary Baptist Church, Mt. Canaan Full Gospel Baptist Church or New Hope Baptist Church, you may be eligible for this program to help you lower your blood pressure.

What is Let’s Get Down 35211?

Let’s Get Down 35211 is a 12-week, community-based high blood pressure management program that provides eligible participants with peer support from trained health coaches, an electronic blood pressure monitor and a group class led by a healthcare provider on strategies for taking blood pressure medication as prescribed. The Let’s Get Down 35211 Program is based at Greater Shiloh Missionary Baptist Church, Mt. Canaan Full Gospel Church and New Hope Baptist Church.

Why focus on the 35211 zip code?

The largest health system in Jefferson County revealed the 35211 zip code as having the highest number of patients with uncontrolled high blood pressure with a diagnosis of stroke or heart failure, two major, costly complications of uncontrolled high blood pressure. A 2017 needs/assets assessment of the 35211 zip code identified the need for health education to increase medication adherence and the availability of trusted faith-partners to support healthy behaviors.

Why focus on high blood pressure management?

High blood pressure is the leading risk factor for cardiovascular disease. Over 41% of adults in Alabama have high blood pressure. Among adults with hypertension nationally, only 52% have their blood pressure controlled.

Who is supporting the Let’s Get Down 35211 Program?

Faith partners, Greater Shiloh Missionary Baptist Church, Mt. Canaan Full Gospel Church and New Hope Baptist Church, will provide space for blood pressure screening and program activities and promote the Let’s Get Down 35211 Program to their church members and the 35211 zip code community. Connection Health, Inc. will provide community health coaches to screen residents for high blood pressure, recruit participants and coach screened eligible participants through the 12-week high blood pressure management program. Let’s Get Down 35211 is funded by the Jefferson County Department of Health and the Community Foundation of Greater Birmingham.

Screening Eligibility

  • Age ≥ 19 years
  • Resident of Jefferson County, Alabama

Program Eligibility

  • Screening blood pressure ≥ 130 mmHg systolic and/or ≥ 80 mmHg diastolic AND self-reported current high blood pressure diagnosis from a provider
  • Member of Greater Shiloh Missionary Baptist Church, Mt. Canaan Full Gospel Church or New Hope Baptist Church and a resident of Jefferson County, Alabama or resident of the 35211 zip code
  • US citizen or National based on signed self-status
  • Intent to reside in Jefferson County for next 6 months
  • Not pregnant or planning pregnancy in next 6 months
  • Not currently participating in a high blood pressure research study*
  • Willing to participate in 3 in-person data collection visits
  • Willing to work with a community health worker
  • Willing to use personal cell phone or landline to work with a community health worker
  • No restriction on participation in physical activity per health care provider
*Will be determined on a case-by-case basis
