Note:  JCDH has changed the brand of fentanyl test strips it is distributing. It is very important that you watch the updated training video and view the instruction sheet as the instructions for use have changed.

All Centers are open 7:45am to 4:30pm. Phone lines for clinic appointments open at 7:00am Monday through Friday.
