13-302.11 Store food wrapped, in packages or in covered containers to prevent contamination. // Unpackaged and uncovered food such as patties inside deep freeze.
14-501.11(A) Maintain equipment in good repair (general). // Deep freezer door not correctly attached to base and repaired with duct tape. Repair or replace damaged deep freeze door.
42-301.15 Wash hands in a hand washing sink or approved automatic handwashing facility. // Hand washing was not being performed at the hand sink due to the water being shut off. 5-202-12 Provide water at a temperature of at least 100 F through a mixing valve or combination faucet at handwashing sinks. // No water available at hand washing sink at start of inspection. COS the valves were turned off. PIC stated that valves were shut off the previous week due to a leak.
16-202.14 Provide tight fitting and self-closing doors on restrooms except where toilet room is located outside of food establishment. (Exception: ADPH—does not apply to toilets used by children in a daycare center) 6-301.14 Post sign at all hand sinks that notifies employees to wash hands. 5-205.15(B) Repair condensate leak in refrigeration units. // Condensate leak in walk in cooler dripping into trashcan.
26-501.12 Clean facilities (floors, walls, ceilings, ceiling/wall vent covers, fixtures, etc.) as often as necessary to maintain cleanliness. // Accumulation of grime/grease/residue on kitchen wall and pipes near fryers and three compartment sink. Accumulation of dust on ventilation throughout facility, such as unit in corner near fryers. 6-501.11 Maintain physical facilities in good repair (floors, walls, ceilings, ceiling tiles, attached fixtures, etc.). // Repair or replace damaged floor tile throughout facility. Repair or replace ceiling holes/damaged ceiling throughout facility, such as in hallway near restroom.
16-304.11 Provide mechanical ventilation of sufficient capacity to keep rooms free of excessive heat, steam, condensation, vapors, smoke, fumes, and obnoxious odors. // No exhaust fan in restroom.
