Planning a Visit to JCDH? Here's What to Expect..

JCDH works diligently to protect the safety of vistors and staff. In order to prevent the spread of COVID-19,  all JCDH visitors are required to wear a cloth face covering upon entering the facility. In addition, JCDH conducts a screening of each visitor before permitting entrance into the building. Below are questions that each person is asked.

  1. Have you been tested for COVID-19 in the past 2 weeks?
  2. Have you been in contact with someone under investigation for, or with a confirmed case of COVID-19?
  3. Have you or a family member been instructed to self-quarantine?
  4. Do you have symptoms of illness – fever, cough, shortness of breath, chills, repeated shaking with chills, muscle pain, headache, sore throat, new loss of taste or smell.

Hours of Operation:
All Centers are open 7:45am to 4:30pm. Phone lines for clinic appointments open at 7:00am Monday through Friday.
